Why it happens

The shoulder blade is the foundation.

Your shoulder is a ball balancing on a shallow socket, and that socket rides on your shoulder blade. When the blade doesn't glide well, because the mid-back is stiff or the muscles around it aren't doing their jobs, the ball gets pinched and squeezed on every reach. That is the mechanism behind most shoulder pain we treat.

Treating only the sore spot misses all of that. We restore the platform first, the shoulder blade and mid-back, then rebuild rotator cuff strength on top of it. That is the order that makes relief stick.

Myth vs. reality

Bad information keeps a lot of people hurting longer than they should. Let's clear a few up.

"A torn rotator cuff means surgery."

Many tears, especially wear-related ones, do just as well with structured physical therapy. Surgery has its place, but it's not the default.

"I should stop using it."

Total rest is how shoulders freeze. Guided movement at the right dose is both safe and the fastest way back.

"A cortisone shot will fix it."

An injection can calm things down, but it doesn't restore the mechanics that caused the problem. Pair it with rehab or the pain tends to return.

Good questions

Shoulder pain, answered.

What is shoulder impingement?
It's that pinching feeling when tendons get squeezed in the space above the ball of the shoulder, usually because the shoulder blade isn't moving well. Restore the mechanics and the space opens back up.
What about frozen shoulder?
Frozen shoulder is real and follows its own timeline, but hands-on care and the right exercise meaningfully improve motion and comfort at every stage.
How should I sleep with shoulder pain?
Try the other side with a pillow supporting the sore arm, or on your back with a small pillow under the elbow. If nights are the worst part, tell your therapist, since that shapes the plan.
Do I need an X-ray or MRI first?
Usually not. A thorough movement exam tells us how to treat most shoulders. If your presentation suggests imaging is needed, we'll say so and point you to the right doctor.
How long until I can reach overhead again?
It depends on the cause and how long it's been going on. Most patients feel real progress within a few weeks, and your therapist will map out an honest timeline at your first visit.
