From your first call to your first visit, here's how it works.
Have your insurance card ready when you reach out. For most plans you don't need a physician referral to start physical therapy in Illinois. Medicare, workers' comp and some commercial plans do require one for coverage, and we'll confirm yours before your first visit.
We contact your insurance company and determine your coverage based on your individual policy, before your second visit, so there are no surprises.
You'll know exactly what your plan covers and what, if anything, you owe. No insurance? We offer a self-pay rate, due at time of service.
A self-pay evaluation is $180, then $160 per treatment visit, or $1,500 for a 10-visit package. Due at the time of service. Cash, check, debit, and all major cards accepted. See the full breakdown on our pricing page.
